A long-awaited internal review into the handling of the case of Hilman Jordan outlined how court-ordered protocols were ignored. Yes, after Mr. Jordan tested positive for a drug that triggered his mental delusions, a court should have been notified, and he should have been confined again. But, no, the city’s mental-health agency insists, those failures did not result in Bhutto’s death.
